Provider Directory
This directory will help you find
providers that participate with the Health Partners Network. Because our
network is consistently growing and changing, we encourage you to always
verify with your provider that they are currently in network before you
receive services.
The
provider panel is available for viewing in different formats. You
may search the database (below) or download a complete listing,

Panel at a Glance, as a Microsoft Word (five-page) document that
includes both facilities and physician providers.

Most health
plans will use the Health Partners network in combination with another
network. If this is the case, the name and logo for the affiliate
network should be listed on the ID card, in addition to the
Health Partners information.
The Health Partners network is commonly combined
with our affiliate network,
1st Medical Network,
for expanded state-wide coverage or it can be
combined with other PPO networks depending upon
the unique need of the health plan.
