What is a CIN?
In today’s evolving health care landscape, there is increasing emphasis on providers working more closely together to improve patient outcomes. More providers are joining Clinically Integrated Networks (CIN), which are collections of health care providers such as hospitals, physician groups and others that collaborate to improve the quality of care and lower costs. A CIN develops, negotiates, enters, and administers contracts with employers, payers and other parties to deliver health care services to their employees or other covered individuals, and the services are delivered by providers in the CIN.
Health Partners Network (HP2)
At the Northeast Georgia Health Partners Network — which is a wholly-owned subsidiary of the Northeast Georgia Health System (NGHS) — we developed a CIN called HP2. HP2 includes physicians and advanced practitioners from Northeast Georgia Medical Center, The Longstreet Clinic, The Heart Center, Northeast Georgia Physicians Group, and other independent practices in the north Georgia area.
Through this network, HP2 partners can pursue performance–based contracting with insurance companies at a community level instead of negotiating these types of contracts on their own.
HP2 creates a mechanism for local and regional clinicians to align themselves within a network of providers. The goal is to become an indispensable care network to payers, patients and providers across northeast Georgia. HP2 assists providers in participating in value-based contracting.
This network has positively influenced care and cost outcomes for patients in a number of health plans.
HP2 is actively pursuing additional arrangements that encourage physicians to deliver exceptional patient care, efficiently and effectively while improving outcomes and reducing the cost. The Triple Aim of healthcare.
HP2 aligns independent, employed and specialized physicians and hospitals in one organization and enhances opportunity for reimbursement for demonstrated quality and cost savings. HP2 is the logical entity to lead population health management across the northeast region of Georgia.
